Sharing Patient Data

THE FOLLOWING INFORMATION PROVIDES DETAILS ABOUT 3 DIFFERENT SCHEMES THAT PATIENTS CAN CURRENTLY CHOOSE TO OPT OUT OF, PLEASE READ CAREFULLY.


SUMMARY CARE RECORD

The Summary Care Record  (SCR) has been introduced to to help healthcare staff  provide a patient with safe treatment when they need  care in an emergency or when the GP practice is closed.  Initially it will contain  your name, dob, nhs number your medications, allergies and any bad reactions to medicines that you have had.

Your information will be extracted from practices such as ours and held on central NHS databases. For further information please visist www.nhscarerecords.nhs.uk or call 0845 300 6016

IF YOU WISH TO HAVE A SUMMARY CARE RECORD.

You do not need to do anything.  This will happen automatically.  Healthcare staff will ask your permission every time they look at your Summary Care Records.

IF YOU DO NOT WISH TO HAVE A SUMMARY CARE RECORD.

If you do not want a Summary Care Record, you need to let us know by filling in and returning an opt-out form.  These can be obtained from reception.


CARE DATA

A leaflet drop was organised by NHS England to all households called “Better Information Means Better Care.”  This leaflet was to raise public awareness about how information from medical records are used, we have briefly summarised this information below and listed contact details should you wish to find out more about the care.data scheme.

  • Care.data is an NHS England national programme.
  • It collects both identifiable and non-identifiable patient information from GP practices
  • It will include coded data such as diagnosis, drugs and lab results
  • InformatIon is managed and anonymised by the Health and Social Care Information Centre
  • This information will be used for planning and research

If you do not want information shared outside your GP practice you need to let us know by filling in and returning an opt-out form.  These can be obtained from reception.


CENTRAL CARE RECORD

This is a locally shared medical record containing allergies, adverse reactions, medication, diagnosis and diagnositic results.  It is only available in Birmingham, Sandwell and Solihull.  This brings relevant health and social care information held on various computers together for example at the moment your GP records are not automatically available when you have a hospital appointment which means that you have to repeat your information to whoever is treating you.  Sensative information such as marital status, IVF treatment, sexual health, complaints, HIV and AIDS, will not be available through the central care record.

In each case when you meet with health and social care staff they will ask your consent before accessing your records, this will happen except in certain situations for instance in an emergency or if you are unconscious. To find out more information please visit
